Background: Lung damage facilitates the colonization of pathogenic bacteria in lower respiratory tract of COPD patients that induces inflammation of which indeed exacerbation, thus contributing to the progression of obstruction which is associated with increased levels of IL-8 and number of bacterial colonization in respiratory tract. This study aimed to determine the relationship between levels of IL-8 sputum with bacterial colonization of the lower respiratory tract and lung function in stable COPD patients.
Methods: Cross sectional study of 52 stable COPD male subjects based on GOLD 2013 in Dr. M. Djamil hospital Padang, Balai Pengobatan Penyakit Paru (BP4) Lubuk Alung Padang Pariaman and laboratorium of microbiology Faculty of Medicine University of Andalas between October 2012 – June 2013. Sputum was collected by spontaneously or induction using hypertonic saline 3% inhalation. Interleukin 8 in sputum was assest with ELISA. Microbiology test was perform to identify potential pathogen microorganism (PPM) using microbat technique and bacterial load in colony forming units/ml (CFU/ml).
Results: The average of age was (67.98±7.18), most of subjects grade III COPD (55.77%). Mean of sputum IL-8 was 656.42 pg/ml, microbiology test found out 65.40% patient had PPM, of which P. auruginosa (38.24%) was the most common. There were association between sputum level of IL-8 and bacterial pattern (p=0.02), IL-8 level had no association with number of bacterial load (p= 0.78) and grade of COPD( p=0.92).
Conclusion: There was assosiation between IL-8 level with bacterial pattern but no assosiation with number of bacterial load and grade of COPD . Colonization of PPM in lower respiratory tract was found in more than a half of severe COPD. (J Respir Indo. 2015; 35: 39-45)
